In 1854, a soldier named Puang, gravely injured in battle, encounters a mysterious ghostly woman clad in black. She offers him a sinister pact: his survival in exchange for becoming her host. Desperate, Puang agrees, and the ghost removes her tongue, forcing it into his mouth, thus binding him to her will. Known as Puang's ghoul, this malevolent entity has since used Puang as a vessel to claim countless victims over the years. Fast forward to 1975, three years after the events of the first film where Puang's ghoul was responsible for Yam's death, Yak has dedicated his life to hunting supernatural entities. Consumed by the desire to avenge his sister's tragic demise, Yak abandons everything to pursue the ghoul relentlessly. He is joined by his steadfast companion, Sarge, who miraculously survived being shot during the earlier events. Together, they embark on a perilous mission to confront the evil force that has wreaked havoc for over a century.
